What Are The Causes Of Achilles Tendon Injuries?

The Achilles Tendon is usually subjected to three common injuries:

  1. Achilles Tendinitis (Achilles Tendon inflammation)
  2. Achilles Tendinosis/Tendonopathy (Achilles Tendon degeneration)
  3. Retrocalneal Bursitis

Over time, the tendon experiences thickening and swelling, culminating in a chronic condition that becomes more challenging to mitigate if left unchecked. Patients typically report stiffness in the heel or ankle area upon waking. In the case of Achilles tendinitis, the discomfort tends to subside with exercise, only to resurface and become painful later. Under microscopic examination, specific cells brought in by the body to control inflammation and aid tendon repair are evident in these inflammatory conditions. At this phase, the recovery time is relatively short. However, if the condition remains unaddressed for over six months, it may evolve into Achilles Tendinosis (see below), which could require a recovery period of 6-12 months.
It’s not uncommon for us at Heel Pain Clinics of Adelaide to see patients with an Achilles Tendon injury that has troubled them for over half a year. The structural integrity of the tendon completely shifts under such circumstances, making it significantly harder to manage compared to Achilles Tendinitis.

Achilles Tendonopathy, also referred to as Achilles Tendinosis, represents the advanced stage of chronic Achilles Tendinitis, and interestingly, it’s not accompanied by inflammation. Instead, patients with this condition exhibit symptoms such as scarring, thickening, and/or microtear breakdown of the tendon. While there is typically no redness or warmth in the surrounding tissue, the area can be sensitive and cause discomfort when touched.

At Heel Pain Clinics of Adelaide we occasionally see Retrocalcaneal Bursitis. It refers to inflammation within the lubricating fluid like sac, which lies between the Achilles tendon and the heel bone. It is highly likely that there is the presence of a prominent bony bump on the outside-top border of the back of the heel. This is known as a Haglund’s deformity.
